Admission covers entrance, including the use the showers and the hot spring baths - there are bars of soap and shampoo plus conditioner as well as hairdryers but no Q-tips, lotion or anything like that. You can also use the relaxation areas to sit/lie down. I don't think there's a time limit. There are a couple of restaurants there but you got to pay for food separately. As far as I know, you don't need a reservation to go in - just buy a ticket using the machines outside. You can also use Ikkyu adjacent to Tenzan (1300 yen) if you pay just 100 yen more - it was much quieter than Tenzan when I went but closed on Thursdays.
